Certification

I certify that the information provided in my application is true, accurate, and complete, to the best of my knowledge. I acknowledge that I am the individual who has completed the form and that I am solely responsible for the form, content, and accuracy of any material contained therein. I understand that providing false, inaccurate, or incomplete information may be a violation of state or federal laws, resulting in civil penalties. I agree to promptly notify the Company of any information that may make the contents of my application inaccurate, untrue, or misleading. I assume full responsibility and risk for my use of the Company site and services, as well as for the documents or materials I provide and my conduct on and off the site.

Authorization

I expressly authorize the Company to submit my application and all information I provided to agencies selected by the Company for employment consideration. I understand that the Company does not guarantee the effectiveness, timeliness, or outcome of its services in helping me achieve my employment objectives. The Company is not responsible for any business, employment, hiring, or salary decisions made by agencies, including any reasons for these decisions.

Release

To the fullest extent allowed by law, I release the Company from any and all claims, demands, and damages (including actual, consequential, direct, or indirect damages), whether known or unknown, suspected or unsuspected, disclosed or undisclosed, arising from or in connection with my application, the information provided, or my use of the Company site and services.

Indemnity

I agree to indemnify, hold harmless, and release the Company (including its subsidiaries, affiliates, officers, directors, employees, and agents) from any third-party claims, actions, or demands (including costs, damages, and reasonable legal and accounting fees) resulting from or related to the information I provided in my application, the documents I provided, my use of the services, or any breach of this agreement. I understand that a breach of this agreement may entitle the Company to equitable relief and recovery of its attorney’s fees and costs.

Limitation on Damages

To the fullest extent allowed by law, the Company (including its officers, directors, shareholders, employees, subsidiaries, affiliates, agents, or advertisers) shall not be liable for any indirect damages (including incidental, consequential damages, lost profits, or damages resulting from lost data or employment opportunity) arising under or in connection with the services or my use of the site. In no event shall the Company be liable for direct damages exceeding $100.

Warranty Disclaimers

The site and the services are provided “as is” without any warranties, express or implied. To the fullest extent permitted by law, all warranties, including implied warranties of merchantability or fitness for a particular purpose, are hereby disclaimed.

Applicable Law and Venue

This agreement shall be governed by the laws of the state in which the Company is headquartered. I submit to the exclusive jurisdiction of the courts in that state, and agree that the sole and exclusive venue for any disputes shall be in the county in which the Company is headquartered.

SMS Communication Agreement
  1. Welcome Message: By accessing or using our services, including receiving SMS communications, I agree to comply with and be bound by these Terms and Conditions. If I do not agree, I will refrain from using the services.
  2. Consent for SMS Communication: By consenting to receive SMS communications, I acknowledge and agree to receive text messages from Converdia Health Staffing at the phone number I provide. My consent to receive these messages will not result in the sharing of my information with third parties.
  3. Types of SMS Communication: I understand that I may receive SMS messages related to updates regarding my application status, onboarding materials, or other employment-related updates. Examples of messages include:
    • “Hello, thank you for applying to one of our positions! When would you be available to start?”
    • “Hello, make sure to download our mobile app to see all of the job opportunities we have.”
    • “Hello, are you still searching for your next assignment? If so, I’d love to help!”
  4. Standard Messaging Disclosures: Message and data rates may apply. Standard messaging rates may be charged by my mobile service provider. You can opt-out at any time by texting “STOP” to the number from which I received a message.   5. Privacy: The Company respects my privacy and will only use the information I provide for SMS communication as outlined above. My personal data will not be shared with third parties unless required by law or as necessary to provide the requested service.
  6. Modifications: The Company reserves the right to update or modify these Terms and Conditions at any time. Any changes will be posted on this page with an updated revision date. By continuing to use the services, I agree to be bound by any changes made to these terms.
